Important Notice: Practice Closing and Patient Records

Dr. Jon M. Roberts, DDS, located at 2320 Northpark, Suite C, Columbus, IN 47203, will permanently close the dental practice effective November 26, 2025. Patient records will remain available at the current office during regular business hours until that date. Beginning December 8, 2025, all patient records will be transferred to and maintained by Dr. Anthony Dee, DMD, who will serve as custodian of records and retain them for at least seven (7) years, as required by Indiana law.

After December 8, 2025, patient records may be requested from:
